Monarch High School Athletic Participation Packet

Includes: Athletic Information Sheet, Sportsmanship Policy, Physical Form, Consent and Release Form (Needs

to be notarized), Broward Health Authorization for Release and Consent for Treatment forms.

Please fill out all forms completely. Note: the student's birth certificate only needs to be on file once throughout

high school, but all the other paperwork needs to be filled out yearly. You may fill out some of the forms online, but you will need to print them for signatures and for the phyiscal's completion by a doctor.

Reminder: Student Athletes must maintain a 2.0 Unweighted GPA to participate in athletics.

All obligations must be cleared in order to participate. Check your Virtual Counselor!

Home Game/Match Ticket Price: $4.00 (Football is $5.00)(as determined by the FHSAA)

District games/matches are determined by the state FHSAA.

Closest restrooms for outdoor athletic events are located in the gym lobby.
