                      First Team All-State in Academics for Baseball

2004-2006-Two year baseball player at Nova Southeastern University in the Sunshine State Conference

                      Set the single season saves mark (12 saves) at NSU while competing at the NCAA D2 level

                      Led The Sunshine State Conference in saves (12) senior year

                      Earned All-Sunshine State Conference Team honors senior year

2007-Present- Head baseball coach at Monarch High School

Monarch High School Athletic Participation Packet

Includes: Athletic Information Sheet, Sportsmanship Policy, Physical Form, Consent and Release Form (Needs

to be notarized), Broward Health Authorization for Release and Consent for Treatment forms.

Please fill out all forms completely. Note: the student's birth certificate only needs to be on file once throughout

high school, but all the other paperwork needs to be filled out yearly.
