To play a sport in a Florida High School students must:

1. obtain and complete a FHSAA physical forms

2. obtain and complete the consent form with proof of insurance and a copy of your insurance card attached

3. Have both of these forms notarized, signed by parent, athlete and doctor where indicated

Transfer students and Freshman will be required to show proof of age by showing a birth certificate or passport to the Athletic Director one time during your tenure at MHS.

This is only needed once a year.

Contact your school for information on school insurance. You should get the coverage that will include summer time activities so you can continue training in the summer.
